The project aims at the development and integration of mobile technologies in the management of type 2 diabetes to support patients in achieving their treatment goals and lifestyle modifications by delivering personalized and situational interventions. Moreover, health care professionals will benefit from these modern training and counseling strategies.

Rising numbers of type 2 diabetes patients paralleled by a decline in the number of health care professionals is one of the biggest challenges of health care systems. Many digital health solutions lack the evaluation through clinical studies and thus tend to be additional systems that do not actually facilitate health care processes. From the start, this project includes all relevant stakeholders in the developmental process, thus allowing the development of mobile solutions along real-world pathways to integration in state-of-the-art health care processes. This ensures the generation of a framework within an optimal environment which will be of a true added value for all stakeholders, thereby warranting a long-lasting implementation. Patients that incorporate the mobile technologies in their diabetes management will not only profit from treatment success in diabetes, but the lifestyle changes will also reduce the risk of secondary diseases and thus result in a reduction of costs for the health care system. Additionally, the face-to-face contact time between health care professionals and patients can be used for higher quality and more time-effective treatment consultations, since the health care professionals receive a systematic long-term overview over the patient’s lifestyle and medical parameters in advance. Currently intensive efforts are being made by pharma companies to enhance their digital portfolios regarding type 1 diabetes care, focusing especially data aggregation and visualization. The framework developed in this project can be integrated in their platforms to broaden the focus of their portfolios towards type 2 diabetes and the support of lifestyle modifications.
